Preventive home visit
The preventive home visit is a cooperative project between local authorities and Diakonie Altholstein. The service is aimed at all people aged 65 and over and can be used free of charge, voluntarily and in person.
During the visits, you will receive preventive advice from our advisors on services and structures close to your neighborhood. Our aim is to maintain and promote your quality of life, health and mental well-being.
What we offer
We provide uncomplicated support and advice on the following thoughts, questions and wishes on your part:
- You are thinking about how to manage your old age.
- You are coping well, but want to know what leisure activities and assistance are available.
- You live alone but would like to socialize.
- You need support in everyday life, but want to live independently.
- You need information about financial support options.
We would like to talk to you about your personal questions and wishes at a personal meeting in your home. The meetings can also take place outside your home or together with other people.
We also provide information, advice and mediation on the following topics:
- Meeting places and events in your neighborhood
- Leisure and cultural activities
- Dealing with digital media
- Home help

- Administrative matters
