Palliative care with heart and expertise

When medical care is no longer aimed at curing, but at alleviating symptoms and providing the best possible quality of life in the last phase of life, palliative care becomes a central issue. Diakonie Altholstein supports people with a life-limiting, advanced and progressive illness and their relatives during this stressful time - with a comprehensive range of medical expertise, holistic care and emotional support.

We offer two forms of outpatient palliative care, which are individually adapted depending on the severity of the symptoms: general outpatient palliative care (AAPV) and specialized outpatient palliative care (SAPV).

Quality of life and care in familiar surroundings

Our aim is to accompany people in their familiar surroundings and provide them with the best possible symptom relief so that hospital stays can be avoided. Our multi-professional team, consisting of experienced palliative physicians and palliative specialists, ensures that our patients receive comprehensive care - both physically and psychosocially. This special form of care makes it possible to provide treatment and care in the desired environment until the end. Our team is available to patients and their relatives as well as the care staff.

Support for patients and relatives

Our palliative care is not only aimed at the patients themselves, but also at their relatives. In addition to medical care, we offer emotional support and advice - before, during and after the death of a loved one. By working closely with the outpatient hospice initiative, we provide support that goes far beyond the medical field, including bereavement counseling.

Assumption of costs and financing

The costs of our palliative care are generally covered in full by health insurance companies, both for patients with statutory and private insurance. The prerequisite is a medical prescription, which can be issued by general practitioners, hospitals or palliative care specialists.

Don't hesitate to seek information at an early stage - even if the topic is difficult. Our team of coordinators offers advice for patients and their relatives regardless of whether they are admitted to care and, if necessary, will also come to your home to discuss individual care options.

Maria & Martha Hospice

The Maria&Martha hospice is being built in the Einfeld district of Neumünster and will accommodate up to twelve guests from 2025. Patients with a life-limiting illness will find a final home there, where they will be cared for by palliative physicians, pain therapists, bereavement counselors, nursing staff, kitchen and housekeeping staff and volunteers.
