Shared apartments for people with dementia
Living independently with dementia, with privacy and community - this is made possible by our outpatient assisted living communities. In these, up to 12 tenants live independently in their own apartment, which they can design individually and furnish with their own furniture. All apartments are located on the first floor and have their own bathroom.
Community
The shared apartments have spacious, open communal areas and a kitchen/living room. These are used for communal living, housekeeping and leisure activities. A garden with a terrace invites residents to linger and enjoy outdoor activities. Depending on their wishes, everyone can join in with the community or spend their time in a quieter and more secluded way.
Everyday support
Maintaining and promoting well-being and quality of life despite physical and mental limitations - this is what the attendance staff and volunteers from the Diakonie outpatient service do for the tenants. Seven days a week, around the clock, they help them to cope with everyday life, tailored to their individual needs and wishes. They create an atmosphere in which the tenant feels safe, protected and cared for.
The aims of everyday support are to maintain independence in household management, promote participation in life in a small community and provide support in organizing everyday life.
Services provided by everyday support in cooperation with the tenants:
- Structuring the day
- Provision of orientation aids
- Joint planning of lunch according to the tenants' wishes
- Support with preparing and eating meals
- Cooking together
- Doing the laundry together
- Accompaniment to the shops
- Offering activities for entertainment, stimulation and encouragement
- Support for own activities and plans
- Assistance with health issues
- Advice and assistance with nutrition and living arrangements
- Assistance with safe care in the event of illness and increasing need for care
Opportunities for relatives to be involved
Relatives play an active role in shaping life in the shared apartment. They retain responsibility and are supported for the majority of the care. At regular meetings with the other relatives in the shared apartment, they discuss how to live together. They make joint agreements and decisions on the organization of the shared apartment.
Nursing and home care
In addition to everyday support, you can also book nursing and illness-related services through our nursing service. Our service staff will also be happy to provide you with domestic support in your own apartment. We will be happy to advise you on how to finance these services.

Music, community, participation: Our choirs and dementia networks create spaces for joy, memories and support. Whether singing together or sharing experiences - we promote a life of community and self-determination.

Reliving memories together, singing, dancing and laughing - these are the goals that we achieve with our choir program. The average of 15 participants meet every Friday for two hours to make music under professional guidance and with the support of volunteers. Old folk music and pop songs in particular open up access to long-lost memories and release emotions. The participants' lives are enriched by the community and the music.
The choir sessions are organized and professionally supervised by staff from our Bad Bramstedt and Henstedt-Ulzburg wards and the Bad Segeberg district music school.
Our dementia networks aim to promote structured cooperation between professionals and lay people who advise, support and care for people with dementia and their relatives. The network partners meet regularly to gather information, discuss needs that have arisen and develop new services. Information events and specialist presentations are used to raise public awareness of the topic and publicize services.
In addition to the specialist events, the aim is also to reach people with dementia at an early stage and enable them to participate and be self-determined for as long as possible. Our aim is to create a dementia-friendly climate in the neighborhoods so that people with dementia can participate in general social life for as long as possible, relatives are relieved and many people can develop an accepting and supportive approach to those affected thanks to the skills they have acquired.
